FY 2009/2010 Annual Report Electronic Submission Instructions |

General Overview: The FY 2009/2010 Annual Report is available for submission electronically through the Internet. The Annual Report login screen may be accessed, completed, submitted and printed by going to: http://envoy.libofmich.lib.mi.us A unique identification code and password have been assigned to each Public, Cooperative and LBPH library. They are included in the cover letter and are needed to access the form. The identification code and password should remain confidential and shared only with appropriate persons. No other library will have access to your data as you prepare the Annual Report form. After logging into the website, the user will view a list of sections, which will appear alongside a status column that indicates if the sections are "Incomplete" or "Complete." The user may browse through the various sections of the Annual Report, and may work on any section of the report at any time, and in any order. Many sections provide data that was submitted on last year's Annual Report. For areas that pre-fill such information, please verify that this information is correct before proceeding. There are many edit checks built into the form that require a response. An Error Message indicator, followed by a list of the errors, will appear in red at the top of the page if the data does not meet formatting criteria or if fields are not completed when trying to Save. Simply make corrections to the page and Save again. If the user must obtain the requested data elements to complete that section at a different time, simply choose "Cancel/To List" in order to proceed, and return to that section at a later time. Though this will not "save" the library's information, it will allow the library to proceed with other sections. The data errors have to be corrected and saved in order for the section to be considered "Completed." Please note that some of the built-in edit checks will require the library to enter a note in the notes field (to the right of the data element). A library may use the notes fields as often as desired, but in some cases will be required to provide an explanation to data that is out of range for their library. When all the sections are designated as "Completed," the library will be ready to electronically submit the form to the Library of Michigan. The electronic report must be submitted on or before February 1, 2010. The "Submit" button will appear after all sections are "complete". Please note, however, that the library will no longer be able to make changes to the electronic form after the "Submit" button is accessed. Contact the State Data Coordinator (by email, fax or phone) to make any changes after this point. After submission, library will be prompted to print a certification of information page. The form should be printed, completed and mailed to the Library of Michigan with original signatures by February 1, 2010. This will be kept on file for review during the Library of Michigan's audit by the Office of the Auditor General. It will serve as proof that the electronic form was submitted and that the data truly represents the library's activities and tha the financial information can be verified by audit. The Library of Michigan will automatically be notified immediately after the library electronically submits the Annual Report form. Library of Michigan staff will be watching for the Certification of Information form shortly after submission. Libraries have the option to print and retain a copy of the report for their file or can view the report (in Read Only format) via the web for a limited time. Important Tips to Remember:
