Welcome to the FY 2009/2010 Annual Report website.
The report is available Monday through Friday, 9:00am to 5:00pm, except during periods of database maintenance.
During this period, users may access and save additions and changes to the report.
The data will be backed up nightly.
FY 2009/2010 Annual Report/State
Aid Application must be submitted, and Certification of Information
postmarked, on or before February 1, 2010, to be eligible to receive
State Aid.

Important Information
- All forms include questions pertaining to current information, as well as fiscal year data.
- Pay extra attention to blue help bubbles available to the left of nearly all question numbers, which includes Federal definitions on the Public form.
- Notes fields are for explanations only. Do not pose filing questions, nor make corrections, within the notes fields!
Public Form:
- NEW DEFINITIONS:Part X Library Services now includes revised definitions pertaining to age groups for Children's and Young Adult Programs. READ the corresponding blue help buttons for definitions.
- It is essential to provide accurate data rather than estimates when at all possible. Non-print collections (audio, visual) MUST NOT be included in print materials numbers.
- MeL resources must NOT be included in ANY collection figures.
- ALL revenue received during the fiscal year must be reported.
- Other Operating Income should include all revenue from non-governmental sources (ie: patrons, donations, bequests, etc)
- Employee Benefits include ALL employer paid expenses (ie: FICA, Medicare, Worker's Comp, etc). Zero is not an accepted amount for this field.
